<P>Sorry for the delay on a new version of the questionnaire.</P>

<P>Gordon, Justin & I met on Tuesday 5th Feb to make changes based on comments from Anton made at the previous meeting, and feedback from Gordon's son Robert.</P>

<P>I need to tidy up these changes before I send it round for further comment, and hope to do this at the weekend.</P>
